We appreciate its mechanical qualities\, which are now enhanced by the use of titanium alloys in orthopedics.However\, its biocompatibility depends on its absence of immune and inflammatory responses\, and therefore on its capacity for inertia in the oral cavity. This biocompatibility is related to its relative resistance to corrosion. Recent studies confirm that the presence of bacteria\, as well as titanium particles\, are involved in the ethiopathogenesis of peri-implant diseases. In addition\, the pollution of our environment\, the presence of numerous allergens in our food and in cosmetic products are leading to the appearance of more and more patients with allergic reactions an increasing number of patients with intolerance or hypersensitivity reactions to metals used in implantology and orthopedics. Patients with an allergic background and autoimmune diseases are also patients at high immunological risk when we indication for dental implants.\nZirconia implants\, with their electro-chemical inertia\, are a genuine alternative.  \n\nDr Fabrice BAUDOT: The benefits of the zirconia collar on anterior implant reconstructions\n \n\nInternational speaker and author of many publications\nExclusive practice of laser micro-surgery\nTherapeutic approach based on mini-invasive surgery\nPractitioner of ceramic implantology since 2013\n\nLecture abstract\nTooth and tissue loss in the anterior region is often a very unpleasant experience. Peri-implantitis is an inflammatory disease of infectious origin\, leading to bone loss around an implant\, which may progress to the point of total implant loss. Peri-implantitis affects an average of 25% of implants in 20% of the implanted population\, at 10 years. Practitioners are currently confronted with the problem of peri-implant infections\, which they have little experience of.\nHow can peri-implantitis be detected? How can they be treated? How can they be avoided? These are just some of the questions that are hotly debated by general practitioners and implantologists alike\, who are confronted with this tsunami in their day-to-day work.\nNumerous therapeutic solutions have beeń proposed\, exploiting various mechanisms and fields of physical and chemical action.\nWe will illustrate particularly through several clinical cases the place of lasers in the treatment of peri-implantitis. This conference will present the APARA protocol for the diagnosis\, management and prevention of peri-implant diseases using “laser mono-therapy”. \n\nDr Roland GLAUSER\, D.M.D: Fighting the Tsunami of peri-implantitis by establishing an effective biological firewall around your implant restorations\nDr. Roland Glauser graduated from University of Zürich and was from 1997 to 2006 assistant professor at the Department for Fixed Prosthodontics and Dental Materials\, University of Zürich. Roland Glauser was president of the scientific board of the Swiss Society of Oral Implantology SSOI and he is an active member of the Academy of Osseointegration AO and the European Association for Osseointegration EAO. From 2006 to 2020 Roland Glauser served as guest lecturer and a research fellow holding a clinical associate professor position at the Dental College of Georgia\, Augusta University\, Augusta (GA)\, USA. \nHis research activities mainly focus on tissue integration of oral implants and shortened clinical protocols. Dr. Glauser has published more than 80 articles and textbook chapters on the subject of restorative dentistry and osseointegrated implants. He served as reviewer for the Journal of Biomechanics\, the Journal of Oral Rehabilitation\, the Journal of Clinical Periodontology\, and Clinical Implant Dentistry and Related Research. Roland Glauser lectures extensively all over the world and received Academy Awards for his presentations from the Academy of Osseointegration AO\, the Japan Academy of Gnathology and Occlusion\, the Asian Academy of Osseointegration AAO\, and the Taiwan Formosa Academy of Implant Dentistry FAID. He serves as a certified expert in Oral Implantology in the European Dental Association EDA and within the Foundation for Oral Rehabilitation FO.  Dr. Glauser runs a renowned private clinic in Zürich\, Switzerland. \nLecture abstract\nAccording to the latest consensus workshop of the EFP and AAP\, peri-implantitis is highly prevalent (affecting 22% of dental implants)\, and its management is challenging\, unpredictable\, and associated with significant morbidity due to the lack of long-term successful treatment options. Given this context\, prioritizing the prevention of peri-implantitis through novel approaches seems logical.\nToday\, zirconia tissue-level implants are not only an alternative to titanium fixtures\, but due to their biological\, esthetic\, and long-term behavior the future of dental implant therapy. Research reveals that a mucophilic and textured implant collar surface character is one of the primary factors contributing to the resistance against peri-implantitis. In particular\, a specific collar surface with its unique ability to promote a robust soft-tissue adhesion along its transmucosal portion is key. Hence\, such characteristics result in the formation of an effective defense barrier – a veritable “biological firewall” – preventing the downward migration of bacteria and plaque. Titanium is still the material more familiar among clinicians but zirconia has gained in popularity in the last years. Finally\, in vivo studies were carried out where the levels of bone index contact as well as the histology of the newly formed bone could be determined.\nThe results showed that the mechanical properties are more favorable for titanium given its greater toughness\, better fatigue behavior and better mechanical transfer to bone tissue. However\, implants made of zirconia do not present corrosion\, have greater chemical stability and aesthetic level. When smooth titanium implants are compared with zirconia implants\, the osteointegration of the different implants at 14 weeks does not present statistically significant differences\, but the study reveals that the key factor is the roughness that plays a fundamental role in the osteointegration of the implant. Innovation has occurred in surface finishes\, design\, implant connections and immediate protocols. In addition\, the advent of digital technology in Cad/Cam zirconia prosthetics and guided surgery has allowed us to treat our patients more quickly\, with a better aesthetic result. However\, the prevalence of peri-implantitis (PI) around titanium implants should be a daily clinical concern due to the high incidence of PI. Recent studies report the presence of titanium particles around implants with PI compared to a healthy peri-implant environment. This is a topic that occupies most of our congresses and raises the question of the long-term reliability of our implant treatments. \nThe qualities of zirconium ceramics as a prosthetic restorative material show us on a daily basis an extremely low bacterial colonisation\, and soft tissues which are then a barrier to the underlying infection. Zirconium is a metal which when oxidised becomes a ceramic\, zirconium oxide. This loses all the properties of a metal\, thermal and electrical conductivity\, which contributes greatly to its high biocompatibility with periodontal cells. \nZirconia implants can be a real alternative to titanium for our patients\, especially those with allergies\, autoimmune diseases\, periodontal risk factors and metal intolerance. We also have to take into account the current trend in dentistry towards non-metallic restorations\, and the aesthetic aspect of our restorations in the long term. The absence of oxidation reaction around zirconia implants and the reduction of bacterial plaque are real assets for their long term stability in the oral sphere. With 20 years of hindsight\, the latest generations of one-piece and now two-piece zirconia implants offer mechanical and biological qualities closer to the natural root. In 1991 he obtained his certificate in dental implantology under the able guidance of Prof. Branemark in Gothenborg\, Sweden. Since 1992 Dr. Luettmann has been a partner of L Dental Clinic for metal free dentistry. \nHis focus is on bioesthetic holistic dentistry\, periodontology\, CMD and orthodontics. His experience with ceramic implants started in 1998 under the tutelage of Prof. Sami Sandhaus in Lausanne\, Switzerland and Dr. Ernst Thomke Head of R&D Z-Systems AG\, Switzerland. \nDr. Luettmann is an international speaker on metalfree dentistry and ceramic implantology. This webinar provides an overview of the options available today with ceramic implants to enable every user to realize a safe restoration with ceramic implants even in difficult situations and to benefit from this excellent material. \nCeramic implants made of stable zirconia have found their way into implantology since the 1990s. In the early days\, these implants were mainly only available in one piece and the surface was smooth. \nAlthough we know from a large number of studies that the mucosal behavior around ceramic implants is more stable and firmer than around titanium implant restorations\, this type of restoration remained mostly the preserve of ceramic implant fanatics. \nThe hardships were too great for the standard user. \nFor this reason\, ceramic implants have been a niche product for many years. \nToday\, more and more patients are rejecting metal restorations for a variety of reasons. Understanding the pathophysiology of this disease helps to develop prevention and effective treatment modalities. In this webinar\, we will discuss one of the possible clinical choices that of zirconia ceramic as an implantable material\, the only non-metallic alternative to titanium and its alloys for dental implants having mechanical validation and clinical experience for this use and which could be one of the answers to this problem.

EACim Ambassador for Germany \nProf. Dr. Marcel Wainwright was born in 1969\, graduated 1994 at the Elite University of Aachen (Germany) where he received his doctor´s degree with summa cum laude 1996. He was working in leading associated practices in Germany for more than 19 years and opened in January 2016 in Stockholm (Sweden) the Mirala Clinic. He is Co-inventor of the Intralift\, the Ultrasonic based crestal minimal invasive sinus augmentation and several instruments and techniques based on ultrasonic surgery and is part of the TKW research group (Troedhan\, Kurrek\, Wainwright). \nProf. Wainwright published more than 60 articles and scientific papers\, contributed his work to published books and was part of the dental olympic team 2008 in Beijing and 2012 in London.
